SUBMITTING A CLAIM

Please submit this claim form to initiate Claim process.

Before submitting a Claim form:

Any person who knowingly presents a false or fraudulent claim for payment of a loss or benefit or knowingly presents false information in an application for insurance is guilty of a crime and may be subject to fines and confinement in prison.

Information we request on the form

Insured’s name, phone number, and email
Legal description of the property
Attach a copy of the Policy, or if a final policy has not been issued, a copy of the commitment and signed settlement statement and signed closing disclosures.
Attach any other documents regarding the alleged title defect, and a brief description of the title issue.
