Please submit this claim form to initiate Claim process.

Information we request on the form

  • Insured’s name, phone number, and email
  • Legal description of the property
  • Attach a copy of the Policy, or if a final policy has not been issued, a copy of the commitment and signed settlement statement and signed closing disclosures.
  • Attach any other documents regarding the alleged title defect, and a brief description of the title issue.
